To receive government-subsidised aged care services, you’ll need to have an assessment through My Aged Care. This process helps identify the type and level of support that best suits your needs. 

Services may be available for people aged 65 years or older, or 50 years or older for Aboriginal and Torres Strait Islander people.

Our aged care system

In Australia, aged care is designed to give you choice and control over the care you would like.

The Commonwealth Government’s My Aged Care team assesses your eligibility for aged care services. If you’re eligible, you may receive government funding to access programs that can support your independence as you age. 

Navigating the assessment process

You will need to complete an assessment through My Aged Care to work out the support you need. The Aged Care Assessment Service (ACAS) will assess your eligibility to access services like in-home support or residential care.  

The process can seem overwhelming, but we’re here to answer all your questions.

 
 

The process step by step

  1. Call My Aged Care on 1800 200 422. They will ask you initial questions to assess if you’re eligible.

  2. An assessor from Aged Care Assessment Service (ACAS) will visit you at home to complete your assessment. This assessment will determine the appropriate support services you can receive. The support you are approved for will be outlined in a letter.

  3. Once you have received your letter, you can choose Uniting Communities as your provider. Call our Community Aged Care team on 1800 615 677. We will listen to your needs and suggest services you may find useful.

  4. We will then work with you to develop a care plan and budget for your services. This will be tailored to your needs and the support level you require.

  5. You can then begin receiving services through our Community Aged Care team.
